Vail Health's Anticoagulation Clinic provides convenient care for patients taking warfarin (Coumadin®), heparin and low molecular weight heparin. These medications, which are blood thinners, control the blood's ability to clot and are prescribed to prevent the formation of harmful blood clots. Careful monitoring can prevent bleeding and clotting complications.
 
The Anticoagulation Clinic at the Cardiovascular Center is a training site for pharmacy students from the University of Colorado and is managed by a doctor of pharmacy and a nurse practitioner who:
  • Provide education to prevent complications related to blood thinners.
  • Review drug interactions and lifestyle influences that impact blood thinners.
  • Monitor lab results performed in our three clinic locations, at home or remotely, and adjust blood thinner dosing accordingly.
  • Guide the management of blood thinners for invasive procedures. 

We are accepting patients by appointment only.  

1. Appointments are available 5 days a week at multiple locations.

    • Monday and Tuesday: 8:30 am - 3:00 pm

    • Wednesday and Thursday: 1:00 pm - 4:30 pm

2. During your appointment you can expect to have a member of our clinical team check your INR 

3. All results and medication changes will be called to you between 3:00 pm - 4:30 pm M-F.

4. Home monitoring results should be called in to the clinic (970-471-4948) when within safe ranges.  Your message will be returned the next business day between 3:00pm and 430pm.  

5. Home monitoring results above 5 and/or bleeding need to be called into the hospital (970-476-2451) and ask to talk with the Cardiologist on call.

Vail Health's Anticoagulation Clinic is an Anticoagulation Center of Excellence.

In order to achieve this designation, the Anticoagulation Forum's rigorous assessment program measured Vail Health's practice against five key areas of patient care. The program also provides ongoing access to a rich library of resources that offer guidelines and tools for staying current with the changing field of anticoagulation. 

Participation in this program demonstrates the Vail Health Anticoagulation Clinic's ongoing goal of achieving the highest level of care and the best possible outcomes for our patients on antithrombotic medications.
