Community Health Specialist

The Community Health Specialist helps patients of Vail Health and their families navigate the complex healthcare systems and connect people to available resources. The Community Health Specialist promotes wellness and improves the health of the community by providing patient navigation, health education, social support, and advocacy.
- Develops trusting relationships with patients by providing support and advocacy to help them achieve their health goals.
- Provides navigation services to patients in a professional, empathetic, and culturally responsive manner.
- Manages referrals and collaborates with appropriate agencies to connect individuals to needed services.
- Meets with patients in patient-centered and patient-preferred locations (e.g., hospital, facility, home, community setting). Travel throughout the community is required.
- Implements approved health and social determinants of health screenings with patients.
- Provides education to the public and community partners including outreach activities and coordinating and/or participating in community events.
- Documents patient interactions in the Electronic Medical Record.
- Follows established departmental policies and procedures.
- Availability to flex throughout the Vail Health system, within scope and training, depending on the identified needs of the system.
- Performs other duties as assigned. Must be HIPAA compliant.
- Role models the principals of a Just Culture and Organizational Values.
- Bilingual (Spanish-speaking) required.
- Two years of experience in the healthcare field, public health setting, or customer service preferred.
- Familiarity with medical terminology preferred.
- Valid Driver's license and ability to pass Vail Health’s Department of Motor Vehicle Report criteria required.
- Basic Life Support (BLS) preferred.
- Patient Navigator Training Collaborative (PNCT) Level 1: Health Navigation Fundamentals preferred.
- Patient Navigator Training Collaborative (PNCT) Level 2: Care Coordination for Patient Navigators preferred.
- Must possess, or be able to obtain within 90 days, the computer skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
- Bachelor’s degree in social work, human services, psychology, or related field preferred.

Pay is based upon relevant education and experience per hour.
