About the opportunity:
Accurately processes medical records, including document capture; identifies documentation deficiencies for physicians in accordance with Federal, State, Hospital and Accrediting Body requirements; prepares documents for disclosure in accordance with Federal HIPAA, State and Hospital regulations and policies; Completes documents for State vital statistics within the State prescribed timeframes. Routinely interacts with the public and physicians. Competently performs the duties of a Health Information Management Technician I.
 
 
What you will do:

 

1. Multiple Primary Duties: Competently and routinely performs 2 or more of the following ** Job Functions. “Routine” is defined as doing the function the majority of the time or completing more than 50% of the function, with occasional assistance from others.

2. **Release of Information: On a daily basis, interacts with providers, clinical staff, and the public (patients, insurers, attorneys, State and Federal agencies, etc.) to accommodate requests for copies of patient information. Understands the release of information policies and associated Federal, State, and Hospital policies. Assists with release of information routinely and when on weekend rotation. Obtains appropriate release request document and verifies patient identity prior to release. Refers Amendment and Restriction requests to Director. Accurately logs releases in CIOX’s or hospital accounting of disclosures/logging system. Efficiently prepares copies of requested in paper, CD, PDF, Fax and other approved electronic formats. Accommodates patient and physician requests within same day. Accesses Vail Health's various systems including its outsourced Cloud system to capture the documentation requested. Ensures only the minimum necessary is disclosed in accordance with HIPAA requirements and complies with policies that ensure the medical record retains its value as legal evidence. May handle release of information requests for Shaw Regional Cancer Center. Routinely handles more than 70% of the Department’s monthly requests. May train others to assist with this function.

3. **Deficiency Analysis: For at least 70% of the monthly encounters, verifies the content of the medical record ensuring that patient identification matches on all documents. Reports discrepancies identified within a patient’s encounter to supervisor or Director. Abstracts data from the medical record for compilation of administrative statistics. Assesses the content of the patient record in accordance with Medical Staff Bylaws, Rules and Regulations, State, and Accrediting body requirements. Identifies documentation deficiencies and assigns to the appropriate provider. Annotates deficiencies. Monitors completion of those deficiencies. Identifies mis-scans and poorly scanned documents, reports on these findings and attempts to correct scanning errors. Recognizes and reports unusual circumstances and/or information with possible risk factors to appropriate risk management personnel and reports problems, errors, and discrepancies in dictation and patient records to Director. Interacts with providers to obtain additional information required to complete and/or code the chart. May train others to assist with this function.

4. **Birth Certificates/Paternity: Collects information directly and indirectly from new parents to accurately and completely prepares and submits birth certificate information in accordance with Colorado State Department of Vital Statistics. Assists patients in declaring paternity responsibilities. Interacts with the State as needed to address birth certificate data. Prepares at least 70% of the monthly birth certificates. May train others to assist with this function.

5. **Amendments/Restrictions: Accurately scans amendment or restriction documents to the medical record upon request from the Director.

6 Work Queues: On a daily basis, reviews and rectifies those HIM Department work assignment queues, such as Outpatient, Inpatient, ED/Urg, Physician Orders, Imports transcribed documents and corrects errors, Pending Coding Cases, etc. Notifies the coders daily of the presence or absence of work in the queue.

7. Other Projects: May work on special projects as required, including, but not limited to updating the dictation system with new dictators and removing dictators no longer associated with the Medical Center, Distributing Delinquency/Incomplete Record Notifications to physicians/providers, Training providers in the use of the dictation system, Serving as a Notary, Interacting with Transcription vendor on dictation issues, etc.

8. Schedule: Accommodates a varied work schedule including weekend coverage, especially during Season, to accommodate the work requirements of the Department and patient needs.

9. Customer Interaction: Effectively diffuses angry or unhappy customers.

10. Organization Culture: Supports the philosophy, objectives, and goals of Vail Health and the HIM department. Role models the principles of a Just Culture and positively collaborates with team members to ensure the Department is functioning at optimal performance at all times by willingly assisting others in various capacities to complete the Department’s work and without compromising one’s own work expectations.

11. Compliance: Complies with all Hospital policies including, but not limited to the Compliance Program and Safety Program. Abides by standards of professional ethical conduct. Attends all required in-services and complete ELearning assignments. Identifies and attends training and educational programs conducive to professional growth. Utilizes current literature and educational sessions attended to the benefit of Vail Health and shares new ideas, policies, regulations and other findings with peers and supervisors.

What you will need:
 
Experience:
  • Health Information Management Department experience of no less than 2 years, required.
  • Scanning and/or release of information experience, required.
  • Experience working with the public in a customer service role, required.
  • Successfully completed a course in medical terminology or anatomy, required.
  • Excellent written and oral communication skills and the ability to work independently with minimal supervision, required.
  • Legible penmanship required.

 

License(s):
  • N/A
Certification(s):
  • Registered Health Information Technician (RHIT) or
  • Registered Health Information Administrator (RHIA) preferred
  • Medical Billing or Coding Certificate, preferred

 

Computer / Typing:
  • Use of a computer, keyboard, and mouse and experience with basic Microsoft Office applications, required.
  • Must possess the computer skills necessary to complete work assignments, online learning requirements for job specific competencies, access online forms and policies, complete online benefits enrollment, etc.
  • Must have typing skills of no less than 20 WPM for this position.
  • Ability to search resources and/or Internet to locate physician and healthcare facility information to make appropriate decisions.
  • Competent in accessing the Birth Certificate application (COVIS) and Dictation System.

 

Must have working knowledge of the English language, including reading, writing, and speaking English.
  • Bi-lingual skills, preferred.
 
Education:
  • Some college coursework, preferred.
  • Graduate from accredited school of Health Information Technology or Management, preferred. 
  • College graduate or certificate in other health related degree or certification preferred.

Pay is based upon relevant education and experience per hour.

Hourly Pay:
$22.60$29.67 USD
