Registered Nurse - Nurse Concierge

About the opportunity:

The Medical Concierge RN works with the Vail Health Major Donor Recognition Program to provide assistance to the recipients of this program in meeting their health care needs. The Medical Concierge RN serves as the primary contact for the recipients of the program and acts as a liaison and support for referrals, scheduling, personal attendance, and follow up for health care services.  The Medical Concierge RN is also responsible for coordination and direct care of participants enrolled in the Vail Health 24/7 MD Program.  

In this role you will work 2x 10 hour shifts plus some on-call.

What you will do:
  1. Offers immediate access to recipients enrolled in 24/7 services program, for purposes of referrals to appropriate health care provider or facility.  In 24/7 MD clinic, delivers direct patient care appropriately and according to the Colorado statute.
  2. Coordinates and facilitates timely, safe and efficient care between patients and health care provider and/or facility.  In 24/7 MD clinic, oversees and organizes the daily patient flow to optimize patient care, safety and satisfaction. 
  3. Coordinates scheduling functions, admissions and paperwork process, along with patient and staff.  In 24/7 MD Clinic, responds to telephone patient requests for medications, information, and assistance.  Administers medications, performs treatments, and assists in procedures as needed; consistently adhering to patient safety guidelines. 
  4. Responsible for periodic visitation and rounding of patients presenting to Emergency Department, Operating Room, and In-Patient areas.  In clinic, competently performs healthcare assessment skills, prioritizing skills, and critical thinking skills. 
  5. Maintains appropriate call logs and/or chart documentation as necessary.
  6. Assists with preparation of periodic reports related to 24/7 operations, quality and performance.
  7. Responsible for being on-call after-hours, holidays and weekends as designated by director, for the purpose of providing “round-the-clock” availability to patient population.
  8. Acts as a liaison and patient advocate between Vail Health and 24/7 Program members.
  9. Along with 24/7 team, institutes practice changes based on findings from clinical monitoring, the evaluation process, recommendations, and research data.
  10. Role models the principles of a Just Culture and Organizational Values.
  11. Perform other duties as assigned. Must be HIPAA compliant.

What you will need:
 
Experience:
  • 5 years hospital experience within Vail Health organization is required.
License(s):
  • Licensed as a Registered Nurse in the state of Colorado or from a valid compact state required.
Certification(s):
  • Basic Life Support (BLS) required.
Computer / Typing:
  • Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
Must have working knowledge of the English language, including reading, writing, and speaking English.
 
Education:
  • Minimum of Associate Nursing Degree for positions hired after 07/01/2018.

Pay is based upon relevant education and experience per hour.

Hourly Pay:
$36$59.33 USD
